"We should pay attention to how we can hallucinate with AI." The phenomenon "can happen when AI introduces errors into the distributed cognitive process, but also when AI sustains, affirms, and elaborates on users' own delusional thinking and self-narratives." Dr. Osler analyzed real cases where "generative AI systems became a distributed part of the cognitive processes of someone clinically diagnosed with delusional thinking." AI companions are "immediately accessible and designed to be 'like-minded' through personalization algorithms." The combination of "technological authority and social affirmation creates an ideal environment for delusions to not merely persist but to flourish."

Context¶
The concept of "hallucinating with" AI reframes the hallucination problem: it is not just the AI producing errors, but the AI and user co-constructing a distorted reality through mutual reinforcement. This is the ultimate convergence of hallucination and sycophancy.
Notes¶
While the clinical cases (delusional thinking) are extreme, the mechanism applies to everyday workplace use: AI confirms a user's business assumption, the user takes the confirmation as validation, and the cycle reinforces incorrect beliefs. This is the workplace version of "co-hallucination."
